Overview

The COVID-19 Screening questionnaire is a form used to validate and verify site/office personnel compliance with Alberici COVID-19 policies. Completion of this form is mandatory for all site/office personnel and should be completed at home, online or on mobile device PRIOR to arriving at the project site/office.

Completing the screening questionnaire

To complete the required COVID-19 screening questionnaire

  1. Click the job specific website link or scan the QR code to access the screening questionnaire.

  2. Read and acknowledge the Screening Alert popup by clicking 'I Understand'.

  3. Enter the requested information into the Project Screening form (all fields are required) and then click ‘Submit’.

4. Save your confirmation # or screenshot your QR code as verification of your completion.

Confirming questionnaire completion

When arriving to any jobsite or office be prepared to provide the Alberici team member the confirmation # or QR code you received after successfully completing the Project Screening questionnaire. Upon display of that confirmation, the Alberici team member will issue a site badge and/or allow access to the site/office.
